On the Cover: Mary Lambert�s Patterns of Accident illustrates the �Swiss cheese� model, which helps explain how small failures in health care can result in misdiagnosis and even death. The health care industry, represented by the caduceus, strives to prevent a breakdown in the precariously balanced string of events that is a fundamental element of all patient care situations.

 

An Albuquerque-based artist, Ms Lambert has worked as an editorial illustrator and graphic designer for the past 20 years.
